If you have trouble printing out this bulletin, check your browser page margin settings. You probably have page margins set too large which is cutting off the page.  Try this to correct the problem.  Open Internet Explorer, on the top line menu go to: "File" then "Page Setup", Be sure that page margins are set as follows:  "Top, Left, Right to 0.25", "Bottom to 0.5"  These margin settings should work for most inkjet and laser printers.

AT&T Medical Plan Problem Resolution
 
 
"AT&T has established an escalation process for retirees who are unable to resolve their issues for an AT&T medical plan as administered by UHC (United Health Care)."
 
In the two attached documents you will find a description of the process and the form as provided by AT&T. Print as many copies of the instructions and form as you may need.

These documents are in what is called a PDF form that will print on most PC’s.
 
This procedure will be enhanced over the next couple of months and include the ability for PC’s with Microsoft Word to email the form rather than FAX it. Our goal is also to create a version that can be mailed via US Mail.
