NEW AT&T Summary of Material Modifications (SMM)
Some of us
received the following in an e-mail from AT&T:
“A
new summary of material modifications (SMM) for the AT&T CarePlus – A
Supplemental Medical Plan - is now available in the Benefits section
of
HROneStop
and
access.att.com.
You can access the SMM on
HROneStop (from
work) or
access.att.com
(from home). Go to the Benefits home page and view the SMM in the
Latest Benefits Information section.
You may also request a free printed
copy. Go online to the Summary Plan Descriptions (SPD) section of the
Benefits section of access.att.com, and select the Order a Printed
Copy link for the SMM. Or you may call the AT&T Health Benefits
Enrollment Center toll-free at 1-877-722-0020
to request a copy.”
Some of us
didn’t.
If you are one
who didn’t, click on the
access.att.com link . If you don’t have a password, you can get
one now – click on Retiree Registration. (Record it and save it.)
Click the
Retiree Logon link. If your computer remembered your password, just
click on “Okay”, otherwise, type in your User Name (your email
address) and password. Click the “Remember my password” box and your
computer should remember it for next time,; then click on “Okay”.
Now click on
“Benefits”; then Select “Employment Status”. (Use the arrow at the
side of the window.) I’m “Retired Management”. Now Select a
Subsidiary. If you find yours, click on it. I used “AIT Retired
Management, After 01/01/01” as mine (they don’t list ALDIS CABS).
Click the button that is labeled “Save my selections above and use
them on future visits”. It doesn’t always remember but It might… have
hope! Now click “SUBMIT”.
You should be
able (with some work and a little luck) to find all Summary Plan
Descriptions (SPDs) and Summary of Material Modifications) (SMMs).
C A U T I O N
! Be sure you choose the SPD/SMM that is part of your coverage. At
this writing, there are 3 SMMs under Latest Benefit Information. Only
the CarePlus one refers to Ameritech people. The Medical Plan is for
legacy AT&T retirees and the Savings Plan is for PAC and SNET people
merging into the AT&T plan. You’ll have to read the first page.
